Understanding TMD
One of the most common reasons patients seek oral health care is pain in the mouth and jaw. Orofacial pain often comes from tooth related issues but can also originate from the muscles, joints, or soft tissues that support oral function. One of the more common non-dental sources of orofacial discomfort are temporomandibular disorders (TMD), which are group of conditions that affect the jaw joints and the muscles that control chewing.

What Is TMD?
Temporomandibular disorders refer to problems affecting the jaw joints and surrounding muscles.
Patients with TMD often describe their symptoms as such:
- Aching or stiffness in the jaw, face, or temples
- Clicking, popping, or grinding noises when opening and closing their mouth
- Limited jaw movement or “locking” in place
- Headaches or earaches that don’t resolve with typical treatments
- Pain that worsens with chewing or the wide opening of the mouth
Because these symptoms overlap with dental and oral conditions, careful evaluation is required to pinpoint the exact cause of the discomfort.
Why Accurate Diagnosis Matters
Oral and facial pain can be frustrating and disruptive. Some live with chronic discomfort for years without finding relief. Determining whether pain is due to TMD, an oral lesion, or another cause is critical because each requires very different treatment.
How TMD Is Diagnosed
Unlike oral lesions that can be confirmed with biopsy, TMD is generally diagnosed clinically through a combination of history, examination, and imaging (when needed). Providers look for tenderness in the muscles of the jaw, limited movement, and noises in the joint. Imaging may reveal degenerative changes or dislocation of the joint structures.
Treatment and Management of TMD
The treatment and management of TMD and other forms of chronic oral pain vary depending on the cause. For TMD, many patients benefit from a more conservative approach. This approach may include:
- Resting the jaw and avoiding hard or chewy foods
- Stress reduction and relaxation techniques to decrease muscle tension
- Custom oral appliances such as night guards
- Physical therapy exercises
- Medications prescribed by a provider
- Injections into tender muscles
