Understanding Soft Trays in Oral Care

For many patients, custom-made appliances provide a comfortable and effective way to deliver medications, protect teeth, and/or support healing. One common tool is the soft tray—a flexible, custom-fitted device that covers a patient’s teeth and sometimes the gums.

Soft trays are versatile and can be used in a variety of ways, from applying medicated gels directly to tissues to protecting teeth from acids to managing oral side effects of systemic conditions.

At the Center for Oral Pathology, we often encounter conditions that are best managed when pathology evaluation is combined with supportive therapies like soft trays. We provide diagnostic and treatment services, some of which involve the use of soft trays.

What Are Soft Trays?

A soft tray is a thin, flexible appliance made of medical-grade material designed to fit closely over the teeth and gums. Unlike hard occlusal guards (built to withstand the forces of clenching and grinding), soft trays are meant for comfort and drug delivery. Soft trays can be worn for extended periods of time without discomfort and are often part of home-based treatment plans.

Why Soft Trays?

Soft trays are used in many different clinical scenarios. These often include:

Medication Delivery. Soft trays are frequently used to hold medicated gels in close contact with affected tissues. This method improves absorption and ensures medication stays in place longer than with rinses alone.

Fluoride Therapy. For patients at high risk of cavities, such as those with dry mouth from Sjögren’s syndrome or radiation therapy, trays can be used to apply prescription-strength fluoride.

Protection of Oral Tissues. In some cases, trays may be used as barriers, protecting the soft tissues from irritants.

Benefits of Soft Tray Therapy

There are a variety of benefits of soft tray therapy over other forms of drug delivery and oral protection. Because they are custom-made, soft trays fit securely and comfortably, which increases patient compliance. They also allow for prolonged contact of medication with the tissues, which can significantly improve effectiveness (compared to rinses or gels applied directly).

For patients with xerostomia (dry mouth), soft trays carrying fluoride can protect against cavities in ways that standard toothpaste can’t. For those with autoimmune or inflammatory conditions, medicated trays reduce irritation and provide consistent, targeted therapy.

Another important benefit is patient independence. Once trays are fabricated, patients can use them at home (as directed), which reduces the need for frequent in-office visits.

When Are Soft Trays Not the Best Option?

While soft trays are highly effective in many cases, they aren’t appropriate for every patient or situation. Some individuals may find them difficult or annoying to wear at night or during daily activities. This is an important consideration as, if not used correctly, medication delivery may be inconsistent.

Role of Pathology in Soft Tray Therapy

Soft trays are most effective when they are part of a larger diagnostic and treatment plan. At the Center for Oral Pathology, we provide thorough evaluations that determine the exact condition being treated.

How Are Soft Trays Made?

  1. Clinical Evaluation. The provider determines whether trays are appropriate for the patient’s condition.
  2. Impressions or Digital Scans. Molds or scans of the teeth are taken to ensure an accurate fit.
  3. Laboratory Fabrication. A soft, flexible material is vacuum-formed or pressure-formed over the cast of the teeth to create the tray.
  4. Delivery and Instructions. The provider fits the tray, checks comfort, and instructs the patient on how to use and clean the tray.
  5. Follow-Up. A patient will return periodically to confirm the effectiveness of their tray. The dental provider will adjust as needed.

This process ensures the appliance is personalized for comfort and treatment success.

Long-Term Use and Maintenance

For many patients, soft trays are part of a long-term management plan. It’s important for patients to:

  • Clean trays daily with gentle soap and water (or a recommended cleanser).
  • Store trays in a ventilated case to prevent bacterial growth.
  • Replace trays when they show signs of wear or distortion.
  • Maintain regular follow-up appointments to monitor both the appliance and the underlying condition.

Proper care ensures the trays remain effective and hygienic over time.
