What Is Surgical Excision?

Surgical excision is the complete removal of a lesion or abnormal tissue from the mouth. Unlike an incisional biopsy, in which only a portion of the lesion is sampled, excision involves the removal of the entire lesion (along with a small margin of surrounding healthy tissue when appropriate).

Why Surgical Excision Matters

There are several key benefits that surgical excision provides. These include:

  • A Definitive Diagnosis – Examining the entire lesion allows for accurate evaluation.
  • Immediate Treatment – Many benign lesions are resolved completely by removing them.
  • Patient Reassurance – Patients can rest assured knowing the lesion has been removed and properly evaluated.

Common Lesions That Are Treated with Surgical Excision

Surgical excision is performed for a wide variety of oral conditions, including:

  • Fibromas – Common benign growths caused by chronic irritation. These often appear on the tongue, cheek, or gums
  • Mucoceles and Ranulas – Salivary gland–related swellings that may recur if not fully removed
  • Pyogenic Granulomas – Vascular growths that bleed easily, often on the gums
  • Peripheral Ossifying Fibromas – Reactive gum lesions that require full removal to prevent recurrence
  • Papillomas – Benign growths caused by HPV that appear as small, cauliflower-like lesions

By removing the lesion completely, our team can both resolve the clinical problem and obtain tissue for pathologic evaluation.

The Surgical Excision Process

While the specific steps may vary depending on the type and location of the lesion, the general process of surgical excision includes:

  1. Clinical Evaluation – Our providers determine whether the lesion is a candidate for excision.
  2. Anesthesia – Local anesthesia is generally used to ensure patient comfort.
  3. Removal of the Lesion – The lesion is carefully excised, often with a small margin of surrounding tissue.
  4. Specimen Handling – The tissue is placed in a fixative solution and submitted to our pathology laboratory.
  5. Microscopic Evaluation – Our oral pathologists examine the entire lesion, looking for evidence of benign changes, dysplasia, or malignancy.
  6. Diagnostic Report – A detailed report is generated, outlining the diagnosis and any recommendations for follow-up.

Why Pathology Is So Important

Microscopic evaluation of an excised lesion provides some incredibly important information, which includes:

  • Type of Lesion – Whether it is reactive, inflammatory, infectious, benign, precancerous, or malignant
  • Completeness of Removal – Whether the lesion has been fully excised or if abnormal cells extend to the surgical margins
  • Need for Additional Treatment – Guidance on whether further surgery or follow-up is necessary
  • Potential for Recurrence – Certain lesions have a higher likelihood of returning, which can be anticipated with pathology findings

With this information, treatment can move forward with confidence and clarity.
